Can Your Brain Feel Pain? The Strange Truth Behind Headaches

A headache can feel as though something deep inside your brain is pounding, squeezing, drilling, or attempting to escape through your forehead.

That makes the obvious explanation seem simple: your brain must be hurting.

Brain tissue itself does not feel pain. The brain receives, processes, and interprets pain signals from the rest of the body, but most of the brain’s own tissue lacks the pain-sensitive nerve endings that would allow it to hurt in the way your skin, muscles, or joints can.

So when your head hurts, what exactly is complaining?

Pain begins when specialized nerve endings respond to potentially harmful stimulation and send signals through the nervous system. The brain then interprets those signals as pain.

This creates a strange arrangement:

Your brain is responsible for producing the experience of pain, but the brain tissue doing the interpreting is generally not the tissue that hurts.

It is a little like a security control room that receives alarms from every part of a building but has no alarm sensor inside the control panel itself.

Then Where Does a Headache Come From?

Your head contains many structures that can detect pain.

  • muscles of the head and neck;
  • nerves in the face, scalp, and neck;
  • blood vessels;
  • parts of the membranes surrounding the brain;
  • certain large arteries and venous sinuses;
  • structures associated with the eyes, teeth, nose, and sinuses.

When these pain-sensitive tissues become irritated, stretched, inflamed, compressed, or otherwise activated, they send signals to the brain. The brain interprets those signals, and the result may feel as though the pain is coming from somewhere deep inside your skull.

The location you feel is therefore not always the exact location where the pain signal began.

The Membranes Around the Brain Matter

The brain is surrounded by protective membranes called the meninges.

Unlike brain tissue itself, portions of these membranes contain pain-sensitive nerve fibers. They play an important role in several types of head pain, particularly migraine.

Much of this sensory information travels through the trigeminal nerve, a major nerve responsible for sensation in the face and large areas of the head. Signals from pain-sensitive tissues can then be transmitted into brain regions that process pain.

This helps explain why a migraine can involve more than a sore head. It may also produce nausea and sensitivity to light or sound.

Can Surgeons Touch the Brain Without Causing Pain?

In some types of brain surgery, patients can remain awake during portions of the procedure.

That sounds like material from a horror movie, but it is possible because brain tissue itself is not pain-sensitive. The scalp, skull, membranes, and other surrounding tissues must be properly anesthetized, but touching certain areas of brain tissue does not produce ordinary pain.

The patient may still experience sensations, movements, memories, speech changes, or other effects when specific brain regions are stimulated.

Does a Brain Tumor Hurt?

The fact that brain tissue cannot feel pain does not mean that conditions affecting the brain cannot cause headaches.

A tumor, injury, infection, bleeding, swelling, or change in pressure can affect nearby pain-sensitive tissues. It may stretch membranes, disturb blood vessels, increase pressure, or irritate nerves.

The pain comes from those affected structures, not necessarily from the brain tissue itself.

So Why Does It Feel Like the Pain Is Inside Your Brain?

Because the brain is working with the signals it receives.

Pain-sensitive nerves around the head send information inward. Your brain interprets the location and intensity of those signals, but the perceived location may feel deeper or more centralized than the true source.

This is known broadly as referred or projected pain: where pain is felt does not always perfectly identify where it began.

In other words, your nervous system gives you an alarm.

When a Headache Deserves Medical Attention

Most headaches are not caused by a dangerous condition. However, a sudden, severe, unusual, or rapidly worsening headache should not be ignored.

Medical evaluation is especially important when a headache is associated with symptoms such as weakness, confusion, fainting, seizures, fever, a stiff neck, recent head injury, or major changes in vision or speech.

Secondary headaches can occur when another condition activates pain-sensitive nerve endings in or around the head.

The Strange Truth

Your brain is the organ that allows you to experience every headache you have ever had.

But the brain tissue itself is not the part feeling the pain.

The discomfort usually begins in nerves, blood vessels, muscles, membranes, or other pain-sensitive structures around it.

Your brain receives the message, interprets it, and convinces you that something inside your head has started a construction project without permission.
